What Does Genetic Mean for Endometriosis?

Hereditary means a trait is directly passed from parent to child through a predictable pattern; genetic means genes play a role, which may be more complex and less direct.

Endometriosis is polygenic and multifactorial: many genes each contribute a small amount of risk, and those genes interact with hormonal, immune, and environmental factors. This is different from a simple dominant or recessive inheritance pattern, no single gene causes endometriosis, and carrying certain genetic variants doesn't mean the condition is inevitable.

How Genetic Is Endometriosis, Really? What the Studies Show 

The clearest answer comes from the largest genetic study of endometriosis ever conducted. A 2023 Oxford-led genome-wide association study published in Nature Genetics, analyzing more than 60,600 cases and 701,900 controls, identified 42 distinct genomic regions associated with increased endometriosis risk. Notably, many of these regions overlap with genetic variants linked to other chronic pain conditions, including migraine and back pain, suggesting a shared biological architecture.

Twin studies add another layer of evidence. Research found that identical twins show significantly higher concurrence for endometriosis than fraternal twins, meaning that when one identical twin has the condition, the other is considerably more likely to have it too. This points to a meaningful genetic contribution that can't be explained by shared environment alone (4).

Is Endometriosis Genetically Inherited — and How? 

Yes, endometriosis is genetically inherited in the sense that it clusters in families. If your mother, sister, or daughter has been diagnosed, your own risk is approximately 7 times higher than that of someone without a first-degree relative with the condition. That's a meaningful elevation, and it's worth taking seriously.

But the inheritance model isn't straightforward. Because endometriosis is polygenic, many genes contribute in small increments rather than a single gene driving the outcome. Those variants then interact with hormonal and environmental triggers, meaning family history increases your risk without making the condition inevitable.

Beyond Genes: Hormonal and Immune Factors That Interact With Genetic Risk
